pplegate Medical Staffing, LLC, has rigid standards of professionalism, experience and knowledge for its contractor agents. On this page you can find out the process of joining the Applegate team.

Important:

Read this entire page. No skipping. You will be expected to know everything and be ready at every stage in the process.

If you think you have what it takes, let’s become prepared, starting with 3 main things.

Online Application

First, to get started, fill out the online form. Choose the appropriate title for your qualifications and licenses.

Experience

Second, be sure to be honest about the qualifications. You must have a minimum of 1 year of provable experience.

Resume

And Third, we will need a resume from you. At the end of the online application you will be given a chance to attach your resume. Please DO NOT fax us your resume. Adobe PDF (.pdf) format only.

If you do not have your resume in .pdf format, convert it before going any further. This is the only format which will be accepted. Information on file conversion can be found in the Resources section of this site.

There are several methods to continue.

  1. Use the online form found here. It will allow you, at the end, to submit a resume attachment in .pdf format only (see above).

  2. Download the .pdf of our application, print it and fill it out. Then you can email it to us, and attach a resume (.pdf only).

  3. Download the fillable-form .pdf. This requires the software Adobe Reader. You can find it in the Resources section of this site. After filling it out, sign it with the digital signature feature, save it and email it to us along with a resume in .pdf format (only).

If you are called or emailed for a phone interview, a time will be set up that will be convenient for both parties.

If you are scheduled for an in-person interview, below is a list of items to have with you.

Potential Contractors: These are the items you should have available when you meet with us.

1. Driver’s License (original)

2. SS Card (original)

3. Current CPR Card (original)

4. Current applicable license[s] (RN, LVN, MA, CNA, etc., from the state of TX — original)

5. Additionally, copies (or originals) of Certificates of Completion, certifications, or anything else that verifies your abilities (ACLS, IV tech, PALS, etc.)

6. Banking information for automatic deposit

7. Proof of being up to date on Hepatitis A and Hepatitis B vaccinations

8. Proof of TB test performed and passed within the last 1 year
